How to Get Google Ads to Find Real Patients, Not Just Leads

If your front desk says half your Google Ads leads are a waste of time, the problem is what you are telling Google to look for. Here is the fix.

If your front desk is telling you half your Google Ads leads are a waste of time, the problem is not Google Ads. It is what you are telling Google Ads to look for.

Why "leads" is the wrong thing to optimize for

Most practices measure their campaigns by leads: form fills, phone calls, booking requests. That sounds reasonable. But a lead is not a patient. A lead is someone who typed their name into a box.

Here is what happens inside the account. You tell Google that a conversion is a form submission. Google takes that literally and goes looking for more people who are likely to submit a form.

It does not know that some of those people had already booked with the practice down the road. It does not know that others are calling every dentist in the area asking who has the cheapest whitening. It does not know that some were spam. All it knows is that a form was filled in, so it finds you more people just like them.

The real question is not how to get more leads. It is how to get Google to stop chasing leads and start chasing patients.

The fix: show Google what a real patient looks like

Instead of telling Google that a form fill is a win, tell it what happened afterwards. Did this person book? Did they show up? Did they become a paying patient? You take that outcome from your own booking system and feed it back into Google Ads.

Think of it like training a new receptionist. If you say "good job" every time the phone rings, they will think every caller is a win, including the wrong numbers and the price shoppers. If you tell them which calls turned into booked patients, they get sharp quickly. Google's bidding learns the same way.

This is called offline conversion tracking, and you may also see it called enhanced conversions for leads. Once it is running, Google stops optimizing for anyone who will fill in a form and starts optimizing for people who look like your booked, paying patients.

How to set it up

  1. Define a real patient. For most practices that is someone who booked an appointment and showed up. Some only count a patient once a deposit is paid. Pick the definition that matches a real win for your practice.
  2. Capture the click ID. Google attaches a unique code, called a GCLID, to every ad click. Your website form or call tracking software needs to capture it and store it against that patient's record.
  3. Connect your booking system to Google Ads. When someone with a stored click ID books and shows up, that outcome is uploaded back into Google Ads, either through a direct integration or a simple weekly import.
  4. Make it your primary conversion. This is the step people skip. Set the new "real patient" action as the primary conversion in place of the form fill. That is the instruction that tells Google's bidding what to chase.

Then give it a few weeks to relearn. Lead volume may look different, and sometimes it drops a little. But the leads that do come in turn into patients at a noticeably higher rate, and that is the trade you want.

Before you start

None of this holds up if your basic conversion tracking is shaky. If Google can't reliably tell when someone filled in a form or called you, fix that first.

If you want me to look at your account and tell you what is costing you real patients, book a strategy call.
